Job summary

The Social Worker at St. Tammany Health System intervenes with patients who have complex psychosocial needs, assists in determining eligibility for social programs, and coordinates care planning with an interdisciplinary team.

Qualifications

• LMSW or higher licensure required; LCSW preferred.
• Three or more years hospital social work or protective services experience preferred.
• Experience in psychosocial and therapeutic counseling preferred.
• Working knowledge of case management, payors and reimbursement.
• Effective written and verbal communication; professional maturity.
• Basic understanding of Medicare, Medicaid, and Managed Care regulations.
• Organized and able to establish priorities and meet deadlines.
• Self-motivated; requires minimal supervision.
• Ability to work under pressure; demonstrates creativity and flexibility.
• Maintains strict confidentiality of all information and documents.
• Familiarity with community resources.

JOB DESCRIPTION AND POSITION REQUIREMENTS

Scheduled Weekly Hours: 40

Work Hours Monday - Friday 800-1630.

JOB SUMMARY:

The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.

MINIMUM QUALIFICATIONS:

• LMSW or higher licensure required, LCSW preferred
• Three (3) or more years hospital social work or protective services experience preferred
• Experience in psychosocial and therapeutic counseling preferred
• Working knowledge/experience in case management, payors and reimbursement
• Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity
• Basic understanding of Medicare, Medicaid, and Managed Care regulations
• Organized and efficient in establishing priorities and performing tasks within specific timeframes
• Self-motivated requires minimal supervision
• Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress
• Responsible for maintaining strict confidentiality of all verbal & written information and documents
• Familiarity with and appropriate use of community resources

PHYSICAL DEMANDS:

Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time.Lifting at least 10 pounds is required.
