St. Luke’s University Health Network is seeking an Outpatient Care Manager, Social Worker to deliver psychosocial assessments, care planning and supportive counseling for outpatients and their families.

The role emphasizes collaboration with the care team to address social determinants of health and coordinate resources to prevent avoidable hospitalizations. Responsibilities include education, crisis intervention, and liaison activities with community organizations.

Qualifications

• Master’s degree in Social Work preferred; bachelor’s degree acceptable with 5 years of medical social work experience.
• PA MSW licensure preferred; NJ MSW licensure required; NJ BSW certification required.
• MSW experience in medical social work/case management preferred or similar

Responsibilities

• Provides assessment, care planning and intervention to patients and caregivers, including psychosocial and resource evaluation and planning, advocacy, and crisis intervention as appropriate.
• Collaborates with providers and care teams to facilitate interdisciplinary care and care coordination.
• Investigates insurance benefits and community resources to provide referrals based on patient/caregiver needs.
• Organizes patient care meetings with internal and external team members to evaluate progress and resolve problems affecting outcomes.
• Provides patient/caregiver education related to government mandates/laws and promotes self-management of care needs.
