Flagship Therapy is seeking a dedicated Full Time Outpatient Speech-Language Pathologist to join Surprise Health & Rehabilitation in Surprise, AZ. You will provide therapy across multiple settings, including skilled nursing, independent living, and assisted living facilities, often visiting patient homes.

The role offers a flexible schedule with work-life balance and pays $40–$50 per hour based on experience.

Qualifications

• Licensed Speech-Language Pathologist required.
• Strong interpersonal and communication skills.
• Ability to travel between facilities and patient homes.
• Ability to work independently and within a multidisciplinary team.

Responsibilities

• Conduct assessments and develop individualized treatment plans.
• Provide therapy services to patients in skilled nursing facilities, independent living, and assisted living facilities.
• Monitor patients’ progress and document responses to treatment.
• Collaborate with a multidisciplinary healthcare team to provide comprehensive patient care.
• Educate patients and their families on treatment plans and home exercise programs.
