Job summary

The Sanctuary at The Woodlands is seeking an Outpatient Services Coordinator to develop and manage our outpatient behavioral health programs, ensuring compassionate, patient-centered care. You will provide leadership, supervise Peer Support Specialists, ensure confidentiality and informed consent, and collaborate with the interdisciplinary team to create treatment plans and discharge planning.

Qualifications

• Master’s degree required in Social Work, Counseling, or related field.
• Strong organizational, interpersonal, and communication skills.
• Experience in behavioral health, outpatient services, or IOP preferred.

Responsibilities

• Provide direct supervision to Peer Support Specialists under the guidance of the Director of Outpatient Services.
• Ensure compliance with legal and ethical standards for patient confidentiality and informed consent.
• Establish meaningful treatment plans and aftercare planning.
• Conduct individual and group therapy sessions, incorporating family members when appropriate.
• Participate in interdisciplinary team meetings to review treatment progress and address social and occupational barriers.
• Advocate for patients' rights, coordinate discharge planning, collaborate with external agencies and connect patients with community resources.
• Assist in program evaluation, including needs assessments, SWOT analyses, and quality improvement initiatives.
• Assist with initial intakes and SLUMs Assessment.

Education

Schedule:

1. Full-time, Monday-Friday 8am-5pm
2. Some evening or weekend hours may be required

Key Responsibilities:
