Job summary

Qualifications

• Completion of an LPN program from an accredited school.
• NH LPN license is required.
• Current BLS for Healthcare Providers.

Responsibilities

• Triages patient calls following guidelines and collects data.
• Documents health information, interventions, and outcomes in the EMR.
• Provides health education to patients to promote optimal health.
• Administers prescribed medications per the five rights of medication administration.
• Performs physical and psychosocial assessments and plans nursing care.
• Maintains competency through annual clinical skill reviews.
