Responsibilities

• Triages patient phone calls following established guidelines and prescribed data collection.
• Documents health information and outcomes in the patient EMR.
• Provides health education to patients to promote and maintain optimum health.
• Administers prescribed medications according to the five rights of medication administration.
• Performs physical and psychosocial assessments and plans nursing care accordingly.
• Completes practice and unit specific clinical skill competencies annually.
• Performs other nursing services commensurate with LPN experience and competencies.
