Job summary

ChristianaCare CareVio in Wilmington, DE is hiring an RN Case Manager for outpatient support. This hybrid position requires weekday hours with one evening per week, focusing on coordinating care for patients with complex medical and social needs and guiding transitions to improve outcomes.

Qualifications

• BSN required.
• Active DE RN licensure or compact licensure.
• Case Management Certification within 18 months of eligibility.
• Experience in Utilization Management or Case Management.

Responsibilities

• Perform ongoing telephonic case management activities including assessment, planning, coordination, monitoring, and evaluation.
• Collaborate with patients, caregivers, and care team to develop a member-centered plan of care.
• Develop, implement, and revise case management care plans per eligibility criteria and guidelines.
• Identify barriers to care and coordinate solutions to promote resolution.
• Promote member wellness through advocacy, education, and referrals to community resources.
