Job summary

ChristianaCare CareVio is seeking an experienced RN Case Manager to coordinate care for a complex patient population in an outpatient setting. The role is hybrid with an 8-4:30 schedule and one evening requirement in Wilmington, DE.

Responsibilities include telephonic case management, care planning, transition coordination, and collaboration with a multi-disciplinary team to improve outcomes and reduce readmissions.

Qualifications

• Registered Nurse with BSN required.
• Active DE RN Licensure or Compact Licensure, in good standing
• Ability to obtain a non-compact state RN Licensure as needed
• Active Basic Life Support (BLS) certification
• Relevant experience in Utilization Management or Case Management required.
• Case Management Certification required through CCM or American Case Management Association within 18 months of eligibility.

Responsibilities

• Perform ongoing telephonic case management activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of case managed members.
• Collaborates with patients, caregivers, and multi-disciplinary care team to develop a member-centered plan of care.
• Develop, implement, evaluate, and revise case management care plans according to criteria and members needs.
• Identify barriers to care and participate in strategies to remove barriers and promote resolution.
• Promote member wellness and autonomy through advocacy, education, and referrals to resources.
• Coordinate transitions and collaboration with various care settings for safe, cost-effective care.
• Stratifies and/or validates member level of risk during transitions and interactions.
• Collaborates with providers to coordinate care across PCP, hospital, SNF, or home care.
