Purpose And Scope

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. It may include delegating appropriate tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities
  • Perform all essential functions under supervision and guidance from the Clinical Educator, Preceptor, or collaborating RNs.
  • Collect and analyze patient data pre-, during-, and post-hemodialysis, document findings, and adjust treatment plans accordingly, notifying the team as needed.
  • Assess and document patient/family learning needs to provide ongoing education.
  • Coordinate and deliver safe and effective hemodialysis therapy in collaboration with care staff.
  • Administer medications as prescribed, ensuring documentation of effectiveness.
  • Initiate or assist with emergency responses.
  • Serve as a resource, participate in staff training and orientation.
  • Ensure proper laboratory procedures and reschedule missed collections.
  • Update care plans based on assessments and team collaboration.
  • Educate patients about transplant and treatment options.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned.
Physical Demands and Working Conditions

The role involves heavy lifting, assisting with patient mobility, prolonged standing, and working with equipment up to 200 lbs. The environment is a healthcare setting with potential exposure to infectious materials. Travel may be required.

Supervision

Oversight of care staff after meeting specific experience and training requirements, including at least 9 months as an RN and 3 months in hemodialysis.

Education and Licensure
  • Graduate of an accredited Nursing School
  • Current state licensure
