Job Overview

Must reside in the Mobile, Alabama region. This role manages identified complex/catastrophic patients attributed to the organization and its network of partner providers. Responsibilities include assessing healthcare needs, identifying barriers to care, developing and implementing comprehensive treatment plans in collaboration with the care team, and coordinating services to ensure seamless care across providers, caregivers, community services, and payors. The job adheres to CMSA Standards of Practice for Care Management.

Minimum Requirements

1. Education: Graduate of an accredited nursing school; Bachelor’s degree preferred.
2. Work Experience: Minimum 3 years in a clinical setting, experience with electronic medical records and Microsoft Office, and working in multidisciplinary teams. Experience in case management or care coordination is preferred.
3. Certifications: Current RN license in the state of practice; CCM certification preferred.
4. Knowledge, Skills, and Abilities: Proficiency in computer applications, excellent communication skills, knowledge of managed care and regulatory standards, organizational and time management skills.

Key Responsibilities

• Collaborate with healthcare team, patient, and caregivers to develop and implement treatment plans.
• Assess social determinants of health and incorporate into care plans; refer to social work or community resources as needed.
• Develop cost-effective care plans and coordinate referrals and appointments.
• Provide disease-specific education and collaborate on self-management goals.
• Identify and report quality issues affecting patient outcomes.
• Perform other duties as assigned, ensuring compliance with laws and standards.

Additional Information

Physical demands include exerting up to 20 pounds of force occasionally, with possible exposure to communicable diseases and hazardous materials. The role involves working within a healthcare environment with occupational risks.
