Must reside in the Mobile, Alabama region. This role manages identified complex/catastrophic patients attributed to the organization and its network of partner providers. Responsibilities include assessing healthcare needs, identifying barriers to care, developing and implementing comprehensive treatment plans in collaboration with the care team, and coordinating services to ensure seamless care across providers, caregivers, community services, and payors. The job adheres to CMSA Standards of Practice for Care Management.
Physical demands include exerting up to 20 pounds of force occasionally, with possible exposure to communicable diseases and hazardous materials. The role involves working within a healthcare environment with occupational risks.
