The role involves managing complex patients, assessing healthcare needs, developing and implementing treatment plans, coordinating care with multidisciplinary teams, and ensuring seamless service delivery across the continuum of care. The job adheres to CMSA Standards of Practice for Care Management.

Responsibilities

• Develop and implement coordinated treatment plans with healthcare teams and patients.
• Assess social determinants of health and incorporate them into care plans.
• Coordinate referrals, appointments, and community resources.
• Provide patient education and support self-management goals.
• Identify and report quality issues affecting patient outcomes.

Qualifications

• Graduate of an accredited nursing school; Bachelor's preferred.
• 3+ years clinical experience; RN license in Alabama.
• Experience with electronic medical records, Microsoft Office, and multidisciplinary teams.
• Certifications: Current RN license; CCM preferred.
