Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will assist patients through clinical triage, care coordination, patient education, and ongoing support to improve outcomes and the patient experience.

The role emphasizes triage, coordination of care, and collaboration with a multidisciplinary team to support transitions of care and chronic disease management.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan.
• Valid CPR certification.
• Minimum 1-2 years of clinical experience in a medical or physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, education, and care coordination.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and health specialists.
• Manage medication refills, test results, referrals, and authorizations.
• Identify preventive care needs and assist chronic disease management.
• Help patients access community resources and navigate care barriers.
• Serve as a clinical resource and leader within the care team.
