Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team. You will triage, educate patients, coordinate care, and support transitions that improve outcomes and the patient experience. Work with providers, care managers, pharmacists, and behavioral health specialists to ensure seamless care.

The role emphasizes patient-centered care, collaboration, and ongoing professional growth within Southeast Michigan’s outpatient network.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Active Michigan RN license with CPR certification.
• 1–2 years clinical experience in medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
