Responsibilities

• Deliver PT treatments and create individualized plans of care.

• Educate patients and caregivers to promote self-management at home.
• Complete documentation reflecting care provided and outcomes achieved.
• Monitor patient progress and adjust plans accordingly.
• Supervise PTAs and aides; stay current with professional standards.
