Rehab Associates in Virginia is seeking a full-time Physical Therapist to assess patients, create and execute individualized treatment plans, and coordinate care with a multidisciplinary team. This role emphasizes outpatient orthopedic care, documentation accuracy, and effective communication with patients and providers.

Qualifications

• Graduate of a CAPTE-accredited Doctor of Physical Therapy (DPT) program.
• Current Virginia Physical Therapist license, or eligibility to obtain one before employment.
• Current CPR certification (or ability to obtain upon hire).
• Experience or interest in outpatient orthopedic care, including balance and vestibular rehabilitation.
• Ability to develop and implement individualized treatment plans in an outpatient setting.
• Comfortable working independently while effectively managing a diverse patient caseload.
• Strong communication, clinical reasoning, and patient education skills.
• Proficient with Electronic Medical Records (EMR) systems and standard Microsoft Office applications.

Responsabilités

• Review the Physician's referral and patient records to determine the physical therapy treatment required.
• Provides direct patient care by established protocols and patient care guidelines.
• Documents findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Completes a written plan of care, develops goals, and a comprehensive program to attain goals.
• Implements physical therapy treatment program and provides essential physical therapy treatments.
• Communicates effectively with patients, caregivers, physicians, and insurers.
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.
