Qualifications

• Bachelor's degree in Physical Therapy is required.
• Master's or Doctorate in Physical Therapy is preferred.
• Current AZ PT license or PT Compact license is required.
• BLS certification is required.

Responsibilities

• Implements an individualized treatment plan using evidence-based therapy techniques.
• Evaluates patients and establishes a comprehensive plan of treatment with measurable goals and discharge plan.
• Maintains accurate documentation in patient records and billing within established timeframes.
• Collaborates with the interdisciplinary team to discuss patient status and discharge needs.
• Supervises therapy assistants and provides instruction to staff; contributes to process improvements.
