Job summary

Endeavor Health is seeking a Physical Therapist II to join the Outpatient Physical Rehab Services in Palatine, IL. This full-time role offers varied hours and a focus on patient-centered 1:1 care in a dynamic rehab setting.

Qualifications

• Degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 care with no double booking.
• Assess, treat and care for patients in the assigned area with developmentally appropriate treatment plans.
• Evaluate functional abilities and plan treatment to regain function, increase strength, and reduce pain.
• Administer and instruct patients in the treatment plan and monitor effectiveness.
• Prepare and complete patient records, billing and documentation per guidelines.
• Collaborate with physicians, nursing and staff to ensure optimal patient care.
• Involve patient and family in the plan of care and educate for self-directed programs.
