H2 Health is seeking a Physical Therapist (PT) in Alexandria, KY for full-time, part-time, or PRN roles. You will evaluate patients, develop plans of care, and provide exercises and manual therapy to improve mobility and function in a general outpatient setting.

Qualifications

• DPT or MSPT/MPT/BPT from a CAPTE-accredited program.
• Current Kentucky PT license or eligibility for licensure.
• CPR certification, if applicable.

Responsibilities

• Evaluate patients and develop individualized, evidence-based plans of care.
• Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients on movement, recovery, injury prevention, and home exercise programs.
• Collaborate with clinicians and support staff to deliver coordinated care.
• Complete timely and accurate clinical documentation.
