H2 Health is seeking a Physical Therapist for our outpatient clinic in Live Oak, FL. This full-time role offers a flexible schedule, competitive pay and bonuses, and a team-driven environment focused on improving lives.

You will treat orthopedic, neurological, pre/post-operative, gait, balance, and sports-related conditions while leveraging AI-assisted tools and Raintree EMR to enhance patient care and efficiency.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy program.
• Active state Physical Therapy license or eligibility for licensure.
• Strong communication, interpersonal, and clinical reasoning skills.

Responsibilities

• Deliver personalized, evidence-based therapy to patients with orthopedic, neurological, post-surgical, and sports-related conditions.
• Document care accurately in Raintree EMR.
• Maintain a caseload of approximately 50 visits per week with AI-driven tools to streamline documentation.
• Contribute to a culture of clinical learning and peer support.
