Job summary

Centra Healthcare Solutions is seeking an experienced or newly graduated Physical Therapist/PT for an outpatient facility in the United States. You will assess referred patients, establish and administer treatment plans, and educate patients and families.

This direct hire/permanent position offers an opportunity to join a growing team and provide skilled PT services under physician direction. The role requires graduation from an accredited program, current PT license or active licensure process,

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.

Responsibilities

• Provide direct patient care to assess medical condition and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and physician direction.
• Set realistic goals, document patient condition, and educate patients and families.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
