Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions as ordered by a physician.
• Assess patient needs, plan, evaluate and modify care for physical therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
