Job summary

Qualifications

Responsibilities

• Provide individual, couple, family and group therapy aligned to patient needs.
• Offer crisis intervention and be available 24/7 for current caseload.
• Conduct assessments, diagnose, and monitor treatment plans.
• Provide case management and discharge planning coordination.
• Complete documentation within established timelines.
• Participate in supervision, peer review and quality-improvement activities.
• Engage in community education and outreach to develop referrals.
