Job summary

Oceans Healthcare in Louisiana is seeking an Outpatient Navigator to facilitate transitions from inpatient care to Oceans Outpatient Program after discharge. The role reports to the Director Outpatient Services and collaborates with the Clinical Director to ensure compliant, patient-centered care.

Responsibilities include educating patients about IOP services, coordinating admit paperwork, and following up after the first two weeks in outpatient care.

Qualifications

• Bachelor’s degree in related field or equivalent combination of education and related work experience.
• At least 1 year’s experience working with psychiatric patients preferred.

Responsibilities

• Educates inpatient groups about IOP services and benefits of the program.
• Facilitates the admit process for patients stepping down into the IOP program.
• Coordinates with discharge planning and involves family participation as appropriate.
• Accompanies patients at discharge to the IOP group and follows up after two weeks.
• Responds to patient and family inquiries regarding the outpatient program.
• Communicates concerns to the IOP director in a timely manner.
• Performs other duties and projects as assigned.
