Job summary

Diversity Group is seeking an experienced Registered Nurse for the Patient Aligned Care Team (PACT) in a primary care setting. As an RN, you will provide and supervise direct patient care, administer medications, and coordinate care across the care continuum.

You will collaborate with an interdisciplinary team, lead education and training, and participate in quality improvement initiatives to deliver patient-centered outcomes. A valid nursing license and English proficiency are required.

Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE.
• Current, full, active, and unrestricted nursing registration in the United States.
• Licensure pathways including BSN with experience or MSN for higher levels.

Responsibilities

• Provide and supervise direct patient care using the nursing process.
• Coordinate care focused on patient education, self-management, and satisfaction throughout the continuum of care.
• Administer medications and procedures per established policies and guidelines.
• Collaborate with interdisciplinary team to influence care outcomes.
• Lead nursing education, orientation, and competency development.
• Participate in quality improvement activities and evidence-based practice.
