Job summary

Diversity Group seeks a Patient Aligned Care Team (PACT) Outpatient Staff RN to deliver direct veteran care and care coordination within the Primary Care Department. The nurse guides clinical education, promotes wellness, and manages panels using registries while collaborating with a multidisciplinary team to improve access and outcomes.

Candidates must meet VA nursing qualifications, hold an active state license, and have ambulatory/outpatient experience; this full-time role offers

Qualifications

• English language proficiency and ability to communicate in English per VA requirements.
• Graduate of an accredited nursing program with valid U.S. RN licensure.
• Current license in state; BSN/ADN with clinical experience in ambulatory/outpatient settings.
• Knowledge of evidence-based care and nursing leadership.
• Ability to work with licensed practical nurses/nursing assistants as applicable.

Responsibilities

• Provides direct Veteran care and care coordination within the Primary Care Department.
• Manages assigned patient panels and utilizes registries to ensure access.
• Provides health promotion and coaching on wellness and chronic disease management.
• Directs nursing education, orientation, competencies and quality improvement.
• Demonstrates self-directed goal setting and evidence-based practice.
