Job summary

The U.S. Department of Veterans Affairs seeks a Staff Registered Nurse for the Patient Aligned Care Team (PACT) in Outpatient settings. The RN will provide competent, evidence-based gender-specific care to Veterans and oversee support staff as appropriate within the Primary Care Department.

The role includes direct Veteran care, care coordination, health promotion, and collaboration with an interdisciplinary team to improve outcomes. Licensure and outpatient nursing experience are preferred.

Qualifications

• Must have an RN license valid in the U.S.
• Graduate of an accredited nursing program (ACEN/CCNE)
• English language proficiency required

Responsibilities

• Provides direct Veteran care and care coordination within the Primary Care Department.
• Manages assigned patient panels and ensures appropriate evaluation and access.
• Provides health promotion, education, and coaching on wellness and chronic care management.
• Assists in directing nursing education, orientation, competencies and quality improvement.
• Collaborates with the interdisciplinary team to influence outcomes.
