Qualifications

• Unrestricted licensure in the state of Connecticut
• Board Certification in Family or Internal Medicine (BC/BE)
• Active, unrestricted CSR and DEA or ability to obtain prior to start

Responsibilities

• Physician-led, patient-centered, team-based primary care environment

• Educate and empower patients to take ownership of their health
• Partner with Advanced Practice Clinicians and care teams
