Job summary

Advocate Health in Mequon, WI is seeking a licensed Doctorate of Physical Therapy to join Outpatient Rehabilitation. The role involves evaluating patients, creating individualized treatment plans, and coordinating care with physicians and staff.

The senior clinician will lead clinical problem solving, document progress, and ensure adherence to safety and quality standards while supporting program development in a hospital outpatient setting.

Qualifications

• Requires Doctorate Degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state where you work (BLS may be required).
• Strong communication and organizational skills required for effective patient care.
• Experience and competency in evaluating and treating musculoskeletal conditions.

Responsibilities

• Interprets physician referrals and completes musculoskeletal and neuromuscular evaluations.
• Develops and implements individualized treatment plans and discharge strategies.
• Completes documentation per department policy and billing requirements.
• Mentors staff, collaborates with physicians, and pursues program development.
