Qualifications

Responsibilities

• Interprets physician referrals and completes evaluations.
• Establishes a plan of care and implements treatment with monitoring.
• Plans discharge including home programs and family teaching.
• Documents care per policy and billing guidelines.
• Maintains safety, infection control, and participates in improvement efforts.
• May supervise PT assistants and rehab aides.
