Job summary

Advocate Health in Wisconsin is seeking a full-time Physical Therapist to join the Outpatient Rehabilitation department. A Bachelor's Degree in Physical Therapy and Wisconsin licensure are required, with BLS within 6 months.

The clinic operates Monday through Thursday 6:30 AM to 6:00 PM and Friday 6:30 AM to 5:00 PM, with a schedule including morning and evening hours to improve patient access. A $5,000 sign-on bonus is offered for full-time new hires.

Qualifications

• Bachelor's Degree in Physical Therapy.
• BLS for Healthcare Providers within 6 months (unless determined not required).
• Physical Therapist license issued by the State of Wisconsin.

Responsibilities

• Interprets referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment-based on evaluations and goals.
• Documents progress notes, discharge summaries, and billing information per policy.
