Job summary

Live Well Homecare in Atlanta, GA is seeking a dedicated Physical Therapist to assess, plan, and implement rehabilitative programs that improve mobility, reduce pain, and prevent deformities caused by disease or injury. You will collaborate with physicians to develop and evaluate treatment plans, design personalized activities, and educate patients and families.

Qualifications

• Graduate from an accredited physical therapy program with a Bachelor's, Master's, or Doctorate in Physical Therapy.
• Hold a valid Physical Therapist license and Basic Life Support (BLS) certification.
• Effective communication, customer service focus, team-oriented mindset, and basic computer skills.

Responsibilities

• Collaborate with physicians to develop and evaluate treatment plans, care goals, and discharge strategies.
• Design and adjust personalized treatment activities, providing patient and family education.
• Document evaluations, treatment sessions, and discharge instructions following departmental guidelines.
• Act as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
