Qualifications

• Graduate from an accredited physical therapy program.
• Bachelor's Degree in Physical Therapy required.
• Master's Degree in Physical Therapy required.
• Doctorate in Physical Therapy required.
• All certifications are required upon hire.
• BLS (Basic Life Support) certification required.

Responsibilities

• Patient assessment in collaboration with the physician, establishing plan of care and discharge plans.
• Develops and revises the plan of care with patient/family input; provides education.
• Implements physical therapy treatment plan and selects activities to progress toward goals.
• Documents evaluation and plan of care according to departmental guidelines.
• Discharges instructions and referrals to community resources; maintains infection prevention.
