Qualifications

• Graduate of accredited program in physical therapy.
• New graduate acceptable.
• Physical Therapist License (Washington) or Oregon
• National Provider Identifier required at hire
• Basic Life Support within 2 months of hire
• Basic knowledge of anatomy, physiology, pathology.
• Equipment operation.
• Basic to moderate computer skills to complete documentation.

Responsibilities

• Delivers quality treatment programs to patients ranging in age from pediatric to geriatric in compliance with competency checklists.
• Evaluates the physical status, functional abilities and age-specific needs of patients to determine current level of functioning and goals.
• Develops and implements a treatment plan involving patient, family and social support; coordinates options with the team.
• Advocates to enhance patients' social support and facilitates environmental modifications.
• Completes chart documentation according to the record review checklist and ensures timely, legible notes.
• Presents in-services as appropriate and maintains professional competency through continuing education.
• Maintains safety precautions and adheres to organizational guidelines.
• Coordinates therapy activities with other health care disciplines and contributes to a clean, safe department.
• Demonstrates leadership, teamwork, and professional conduct; adapts to changes and ensures high-quality care.
