Job summary

Kaiser Permanente Northwest is seeking a Physical Therapist to provide patient-centered therapeutic care in collaboration with the health care team at Skyline Medical Offices in Oregon. Responsibilities include assessing patient needs, developing treatment plans, and coordinating care with other disciplines to prevent disability and restore function.

The role requires licensing in Oregon and Washington, completion of an accredited PT program, and ongoing documentation and compliance with safety

Qualifications

• Graduate of accredited program in physical therapy.
• New graduate acceptable.
• Basic knowledge of anatomy, physiology, pathology.
• Basic to moderate computer skills to complete documentation.

Responsibilities

• Delivers quality treatment programs to patients across ages; evaluates status and needs to determine function.
• Develops and implements a treatment plan with patient, family, and social supports.
• Coordinates therapy activities with other health care disciplines.
• Completes chart documentation according to review checklist.
• Provides patient handouts that are effective and appropriate for understanding.
• Maintains professional competency by attending continuing education applicable to the patient population.
• Presents in-services as appropriate.
• Demonstrates leadership and teamwork; maintains confidentiality of patient information.
• Interacts with patients/families/caregivers in a friendly and courteous manner.
