Job summary

Kaiser Permanente is seeking a licensed Physical Therapist to manage patient care under the direction of Rehabilitation Services Director in a labor/management partnership environment.

You will evaluate patients, plan and implement treatments, and coordinate care with other healthcare professionals while ensuring thorough documentation and discharge planning when goals are achieved. This on-site role requires California PT licensure and BLS at hire.

Qualifications

• Graduate of an accredited physical therapy curriculum with a bachelor’s, master’s, or doctoral degree and PT certification.
• California Physical Therapist License, National Provider Identifier, and Basic Life Support at hire.
• Must work in a Labor/Management Partnership environment.

Responsibilities

• Evaluates patients and develops treatment goals and plans.
• Directly treats patients and coordinates with health care personnel; provides education to patient and family; monitors progress toward goals.
• Completes timely documentation of patient management and discharge planning when goals achieved.
