Position Details

We are seeking a part-time Outpatient Physical Therapist to join our rehabilitation team at the Lockport, IL clinic, located within Bo Jackson's Sports Dome. The therapist will plan, implement, and evaluate patient care programs aimed at restoring function, preventing disability, and helping patients reach optimal performance levels.

Responsibilities

• Evaluate patients and gather clinical data to determine diagnoses and treatment plans.
• Set functional, objective goals based on examinations.
• Develop detailed treatment and care plans.
• Provide exceptional patient care and service.
• Maintain positive relationships with clients and colleagues, fostering a respectful work environment.
• Build relationships with patients and the community.
• Communicate with referring physicians and other healthcare providers to ensure consistent care.
• Maintain accurate patient records, logs, and checklists to ensure compliance.
• Protect patient confidentiality in accordance with HIPAA and company policies.
• Communicate professionally in all settings.
• Perform additional duties as assigned.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.
• Current State of Illinois Physical Therapy license (or willingness to obtain).
• CPR certification.
