Ready to join? We are looking for a part-time Outpatient Physical Therapist to join our rehabilitation team. This position will be based at the Lockport, IL clinic, located within the Bo Jackson's Sports Dome. Our PTs plan, implement, and evaluate patient care programs designed to increase, maintain, or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Evaluate patients and gather appropriate information to determine accurate clinical diagnoses and treatment plans.
• Establish appropriate, functional, and objective goals based on clinical examination.
• Devise detailed treatment and care plans for patients.
• Provide exceptional patient care and service.
• Maintain friendly, cordial relations with all clients and employees; foster a positive work atmosphere through respectful communication.
• Cultivate relationships with patients and the community.
• Provide continuous communication with referring physicians and other referral sources to ensure consistent care.
• Maintain patient charts, clinical logs, and checklists necessary for compliance.
• Ensure patient confidentiality regarding medical records in accordance with company policies and HIPAA guidelines.
• Communicate effectively in a professional setting.
• Perform additional duties as required.

Education and/or Experience

• Bachelor’s, Master’s, or Doctorate Degree from an accredited Physical Therapy Program
• Current State of Illinois Physical Therapy license (or willingness to apply)
• CPR Certified

Pay Range: $80,000 - $100,000 annually. Compensation is determined by skills, experience, education, certifications, location, market data, and internal factors. Base pay is only part of the total rewards.
