Job summary

AdventHealth in Palm Coast, FL seeks a licensed Physical Therapist to diagnose, evaluate and treat patients, develop a plan of care, and collaborate with physicians, staff and families to improve function and mobility.

This full-time, day-shift role includes benefits, sign-on bonus up to $10,000 and relocation bonus up to $3,000, with a minimum pay rate of $36/hour. Location: Sports Medicine and Rehab at 60 Memorial Medical Parkway, Palm Coast, FL 32164.

Qualifications

• Education requirements are determined by licensing standards and are required to obtain the applicable license or certificate.
• Doctorate in Physical Therapy preferred for this role.
• Experience treating neurological disorders preferred.

Responsibilities

• Determine physical therapy diagnosis and establish plan of care with referrals when needed.
• Document and bill accurately using standard terminology per policy and guidelines.
• Engage in marketing to physicians and practices to build referral relationships.
• Perform evaluations and re-evaluations using recommended techniques.
• Support community education and outreach to extend clinical expertise.

Job Description

Schedule: Monday - Friday 8am - 5 pm

The anticipated minimum pay rate for this position is $36/hour.

• Determines physical therapy diagnosis and establishes a plan of care, making recommendations for referral if treatment is needed outside the scope of physical therapist practice.
• Documents and charges accurately, completely, and timely using measurable and objective terminology following standards of practice, insurance guidelines and department/hospital policy and procedures.
• Participates in marketing to physicians and physician practices to build referral relationships.
• Performs evaluations and re-evaluations using recommended techniques and standards.
• Participates in marketing to physicians/physician practices to build referral relationships.
• Supports consumer marketing and public awareness through participation in community events, educational lectures/speaking, and/or other extension of clinical expertise to the community.
• Adjusts and adapts to changes in patient census by altering working hours to assist in other centers and programs as approved and observed by the manager or director.
• Practices principles of professional development by maintaining clinical competencies, using best and/or evidence-based practices, and serving as a clinical instructor.
• Oversees the care provided by physical therapist assistants and rehabilitation therapy aides, according to established regulations and practice standards.
• Participates in practice changes as a result of performance improvement activities and supports quality standards and initiatives set by the department.
• Maintains productivity standards as defined by the department, through effective time management and clinical efficiency and adjusts work hours to meet the needs of the patients as required.
• Designs and implements treatment plan of care utilizing practice standards, recommended techniques, modalities, insurance guidelines, and patient needs.
• Assesses patient ability to safely and effectively perform functional mobility skills and evaluates muscle, nerve, joint, and integumentary systems.
• Monitors patient response and progress to treatment, modifying the treatment plan as appropriate.
• Performs other duties as assigned.
