Job summary

AAH is hiring a Physical Therapist for outpatient rehabilitation in Menomonee Falls, Wisconsin. This full-time role requires 40 hours per week, with a preference for four 10-hour days and occasional acute care weekend rotations at Aurora Medical Center - Washington County in Hartford, WI.

Responsibilities include comprehensive evaluations, treatment planning, progress monitoring, discharge planning, and thorough documentation.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Active Wisconsin Physical Therapist license required.
• Basic life support (BLS) certification within 6 months if not already held.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements individual treatment based on evaluation results and goals.
• Monitors outcomes and adjusts treatment plans; provides updates to physicians.
• Plans for and completes patient discharge with home programs and family teaching.
• Completes documentation per department policy including progress notes and discharge summaries.
• Maintains clean, orderly treatment areas and uses modalities and equipment appropriately.
• Participates in performance improvement activities and supports cross-coverage as needed.
• Demonstrates knowledge of age-specific care and ongoing professional development.
