Northwest Rehabilitation & Associates, a member of Therapy Partners Group, is seeking an Outpatient Physical Therapist to join our team in the Salem/Keizer/Dallas area. You will provide high-quality patient care, develop tailored treatment plans, and help patients achieve rehabilitation goals in an outpatient setting.

Requirements include a Doctor of Physical Therapy (DPT), current licensure or eligibility, and the ability to work collaboratively.

Formación

• DPT from an accredited program
• Current state licensure or eligibility for licensure as a Physical Therapist
• Outpatient experience preferred but not required
• Ability to work collaboratively in a team-oriented environment
• Commitment to providing exceptional patient care

Responsabilidades

• Patient evaluation and comprehensive assessments of mobility and functional abilities
• Develop and implement individualized treatment plans based on evaluations and goals
• Apply therapeutic interventions including manual therapy, exercise prescriptions, modalities, and patient education
• Document progress and outcomes accurately in clinic records
• Educate patients and families about rehabilitation processes and home programs
• Monitor progress and adjust plans based on patient response
• Engage in continuing education and professional development to stay current
