Job summary

Northwest Rehabilitation & Associates in the Salem/Keizer area seeks an Outpatient Physical Therapist to provide high-quality care in an outpatient setting. You will evaluate patients, develop individualized treatment plans, implement therapies, and document progress for ongoing rehabilitation goals.

The ideal candidate holds a Doctor of Physical Therapy, active licensure or eligibility in Oregon, and enjoys collaborating with a team.

Qualifications

• DO degree in physical therapy from an accredited program.
• Active licensure in Oregon or eligibility for licensure.
• Outpatient experience is preferred but not required.

Responsibilities

• Patient Evaluation: Assess mobility, function, and conditions.
• Treatment Planning: Create individualized plans based on goals.
• Therapeutic Interventions: Apply manual therapy, exercise, modalities, and education.
• Documentation: Record progress and outcomes in the charting system.
• Patient Education: Instruct on rehab steps, home programs, and prevention.
• Progress Monitoring: Adjust plans based on progress and feedback.
• Professional Development: Stay updated with latest PT research and practices.
