Job summary

Advocate Health is seeking a Certified Lymphedema Therapist to join our Rehab Services team in Morehead. The position involves evaluating patients, selecting assessment techniques, and developing individualized care plans within an outpatient rehabilitation setting.

Qualifications

• Bachelor's degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and develops care plans with appropriate modalities.
• Gathers data from records, physician reports, patient and family.
• Establishes functional goals and provides evidence-based intervention.
• Documents evaluations, progress notes and discharge notes.
• Coordinates patient schedule and discharge planning with team.
• Supervises PTA's and Rehab Tech/Aide as required.
