Job summary

H2 Health in London, Kentucky is seeking a Physical Therapist (PT) for outpatient care. We offer Full-Time, Part-Time or PRN roles, welcoming new graduates and providing strong admin support and access to AI-assisted documentation to reduce burden.

As part of our outpatient team you will evaluate patients, design evidence-based plans, guide therapeutic exercise and functional training, and educate on recovery and home programs.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent CAPTE-accredited program.
• Kentucky PT license or license eligibility.
• CPR certification, if applicable.
• Strong communication and collaboration skills.
• Commitment to patient-centered care.
• New graduates welcome; outpatient experience not required.

Responsibilities

• Evaluate patients and develop individualized, evidence-based plans of care.
• Provide therapeutic exercise, neuromuscular re-education, manual interventions, and functional training.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate patients on recovery, movement, injury prevention, and home exercise programs.
• Collaborate with clinicians and support staff to deliver coordinated care.
• Complete timely and accurate clinical documentation.
