H2 Health is seeking a Physical Therapist (PT) for our Richmond, KY outpatient clinic. Join a team dedicated to helping patients move better, feel stronger, and live healthier lives with evidence-based care and compassionate service.

You’ll evaluate patients, create individualized plans, and collaborate with PTAs to deliver quality, patient-centered rehab. New graduates are encouraged to apply, with access to mentorship and CE opportunities.

Qualifications

• DPT required and licensed to practice in Kentucky.
• Excellent communication and interpersonal skills.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide evidence-based physical therapy interventions to improve mobility, function, strength, and quality of life.
• Treat a variety of orthopedic and musculoskeletal conditions, including post-operative rehab.
• Collaborate with Physical Therapist Assistants and support team members for strong patient outcomes.
• Educate patients and caregivers on treatment plans, home exercises, and injury prevention.
• Accurately document patient evaluations, treatment progression, and outcomes.
• Build meaningful patient relationships with compassionate, patient-centered care.

Skills

Education

Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program

Kentucky Physical Therapist license or eligibility

CPR certification, where applicable
