H2 Health in Berea, KY is seeking a Physical Therapist (PT) for outpatient care. Choose Full-Time, Part-Time, or PRN schedules to fit your life while delivering patient-centered care.

You'll evaluate patients, develop evidence-based plans, and collaborate with a skilled team in a 2,500-square-foot clinic serving diverse musculoskeletal needs. New grads welcome; Kentucky licensure and CPR are required, with strong communication valued.

Qualifications

• DPT or MSPT/MPT/BPT from CAPTE-accredited program
• KY PT license or eligibility
• CPR certification (if applicable)
• New grads welcome; outpatient experience not required

Responsabilités

• Evaluate patients and develop individualized PT plans
• Treat diverse outpatient caseload focused on mobility, strength, function, and activity
• Provide therapeutic exercise, neuromuscular re-education, manual therapy, and functional training
• Educate patients on movement, recovery, injury prevention, and home exercise programs
• Collaborate with clinicians and support staff to coordinate high-quality care
• Track patient progress and adjust treatment plans
• Complete timely and accurate clinical documentation
