Carolina Physical Therapy and Sports Medicine in Rock Hill, SC, seeks a dedicated Physical Therapist to assess patients, develop care plans, and deliver direct therapy. The role emphasizes collaboration with physicians and insurance providers, with a focus on evidence-based rehabilitation.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy program.
• Current state South Carolina license.
• CPR certification.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician's referral and patient records to determine required therapy.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Supervise assistants and rehabilitation technicians during patient care.
• Develop a written plan of care, goals, and a comprehensive program.
• Implement the physical therapy treatment program and essential treatments.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including notes and reports.

Job Description

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

Additional Information

The salary range begins at $75,000 and increases based on relevant experience.Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
