Qualifications

• PharmD or BS Pharmacy degree from a College of Pharmacy accredited by the AACP.
• Current Pharmacist license in Indiana, or the ability to obtain Indiana licensure.

Responsibilities

• Evaluate and interpret medication orders and prescriptions
• Compound and dispense medications in accordance with legal and professional standards
• Educate and counsel patients on medication use, adherence, side effects, and expected outcomes
• Collaborate with providers, payers, and manufacturers to support access to therapy and continuity of care
• Manage prior authorizations and utilization management activities
• Monitor medication safety, effectiveness, adherence, and patient outcomes
• Ensure REMS compliance and applicable accreditation standards
• Support quality improvement initiatives, regulatory reporting, and pharmacy performance metrics
• Identify and resolve medication-related issues to optimize patient care and outcomes
