Job summary

Qualifications include PharmD or BS in Pharmacy, possible ASHP residency, Indiana licensure, and BLS; travel and weekend shifts may be required to support IU Health facilities.

Qualifications

• PharmD or BS in Pharmacy required (prior to 2006).
• ASHP-accredited PGY1 or PGY2 residency may be required.
• 0–3 years of relevant experience.
• Current Indiana Pharmacist license or eligibility for Indiana licensure.
• Board certification or other specialized training may be required.
• Basic Life Support (BLS) certification may be required.
• May require travel to IU Health facilities.
• May require weekend rotations.

Responsibilities

• Provides comprehensive, value-based medication-related care to improve a patient’s quality of life.
• Collaborates with other healthcare professionals to optimize medication therapy outcomes.
• Ensures safety and effectiveness of the medication use process and dispenses medications per order.
• Maintains confidentiality of patient information and regulatory compliance.

Doctor of Pharmacy (PharmD) or B.S. in Pharmacy (prior to 2006)
