Job summary

Penn Medicine Lancaster General Health is seeking an Advanced Practice Provider (APP) to join the Outpatient Palliative Oncology Clinic. The APP will work with medical, radiation, and surgical oncology teams to provide symptom management, advance care planning, and psychosocial support for cancer patients and families.

The role emphasizes collaboration, longitudinal patient relationships, and a biopsychosocial approach to care in a patient‑centered, multidisciplinary setting.

Qualifications

• Valid Certified Registered Nurse Practitioner license in Pennsylvania.
• Certificate of Prescriptive Authority in Pennsylvania.
• Certification by ANCC or other qualifying agency in area of specialty.
• Adult-Gerontology Primary Care or Family Nurse Practitioner certification is required.
• Valid Pennsylvania licensure as a Registered Nurse.

Responsibilities

• Provide outpatient palliative care consultations and follow‑up visits for patients with cancer.
• Assess and manage complex symptoms related to cancer and cancer treatment.
• Facilitate goals‑of‑care conversations and advance care planning.
• Collaborate closely with oncology physicians, nurses, social workers, and supportive care services.
• Partner with inpatient palliative care teams to promote continuity of care during transitions between ambulatory and hospital settings.
• Support patients and caregivers through serious illness using a comprehensive biopsychosocial approach.
• Participate in interdisciplinary care planning and team meetings.
• Assist with development and growth of the outpatient palliative oncology program.
