Job summary

Penn Medicine Lancaster General Health is seeking an Advanced Practice Provider (APP) to join our outpatient palliative oncology clinic. The role focuses on symptom management, advance care planning, and goals-of-care discussions for patients living with cancer, working closely with medical, radiation, surgical oncology, and inpatient palliative care teams.

Qualifications

• Experience in outpatient palliative oncology or related field preferred.
• Certified Nurse Practitioner (CRNP) license in Pennsylvania with Prescriptive Authority.
• Certification by ANCC or equivalent in area of specialty.

Responsibilities

• Provide outpatient palliative care consultations and follow-up visits for cancer patients.
• Assess and manage complex cancer-related symptoms and treatment side effects.
• Facilitate goals-of-care conversations and advance care planning.
• Collaborate with oncology physicians, nurses, social workers, and support services.
• Support continuity of care during transitions between ambulatory and hospital settings.
• Participate in interdisciplinary care planning and team meetings.
